In World War I, a young Basil of Baker Street devises a successful strategy to advance his Belgian squadron, but a booby trap mutilates his face. His lover, nurse Gadget Hackwrench, suggests he grow a mustache to hide his scars. In 1937, Basil watches blues singer Lady Otterbourne perform at a London club, and sees Scarlet Overkill introduce her fiancé Gaston Doyle to her childhood friend, heiress Shank Ridgeway.
Six weeks later in Egypt, Basil encounters his friend Chase, who introduces him to his mother Jenna, a painter, and to a surprising newlywed couple – Shank and Gaston. Others join their honeymoon trip: Shank's maid Fluttershy; Lady and her niece/manager Skye, Shank's schoolfriend; Shank's godmother April Spink with her nurse Mrs. Forcible; Shank's cousin Hiro Hamada, who manages her accounts; and Alchemist Varian. Shank asks Basil for protection from the obsessive Scarlet, who has stalked them to Egypt; he tries to dissuade Scarlet, who shows him she carries a gun.
To escape Scarlet, the group boards the cruise ship S.S. Karnak, though Shank tells Basil she does not trust her guests. During an excursion to Abu Simbel, Chase confesses that he is dating Skye, despite his mother's disapproval; Basil shows interest toward Lady. After a boulder nearly crushes Shank and Gaston, the guests return to the Karnak to discover Scarlet has boarded. Basil, overcome by champagne, confides to Scarlet that he renounced romance after Gadget died in a mortar explosion. Shank goes to bed and Gaston confronts Scarlet, who shoots him in the leg and attempts to shoot herself, but Skye and Chase intervene. They bring Scarlet to Mrs. Forcible, while Varian treats Gaston. The following morning, Fluttershy discovers Shank has been shot in the head, and her valuable necklace has been stolen.
Basil, assisted by Gaston and Chase, interrogates the guests, who each bear a grudge against Shank:
- Fluttershy was to leave Shank's employment to be married, but Shank ended the engagement.
- Varian was engaged to Shank until she left him for Gaston.
- Hiro, who has a gun, was embezzling from Shank.
- Forcible's formerly wealthy family was evicted due to the actions of Shank's father during the Great Depression.
- April is a beneficiary of Shank's will; Basil deduces that she and Forcible are lovers.
- Lady also carries a gun, and was insulted by Shank years ago.
- Jenna finds Shank's necklace, but Basil suspects she resented Shank for introducing Chase to Skye.
Scarlet, however, was monitored by Joy all night. Her gun is dredged from the Nile, wrapped in April's missing scarf and a bloody handkerchief.
Basil reveals that he was hired by Jenna to investigate Skye's suitability to marry Chase. Fluttershy's body is found with money and her throat slit; suspecting that she witnessed Shank's murder and blackmailed the killer, Basil sees a possible witness' outline in Fluttershy's blood spatter. Interrogating Chase with Gaston, Basil deduces that Chase found Shank dead and stole her necklace for financial freedom from his mother, but panicked and placed it in her belongings. He then witnessed Fluttershy's murder, disposing of his coat covered in her blood. Before he can reveal the killer, Chase is shot dead; Basil chases the killer, but only finds the abandoned gun.
Locking the surviving guests in the boat's bar, Basil reveals that the boulder was dislodged by a remorseful Hiro, but that Gaston is Shank's killer with Scarlet as the mastermind – they are still lovers, and arranged Gaston's romance with Shank to inherit her wealth. Having drugged Basil's champagne, Scarlet pretended to shoot Gaston with a blank, and Gaston faked his injury with paint stolen from Jenna. While Scarlet distracted Chase and Skye, Gaston killed Shank then shot his own leg, muffled by April's scarf. Scarlet killed Fluttershy with Varian's scalpel, and Chase with Hiro's gun. Faced with the proof of Gaston's paint-stained handkerchief, Scarlet – knowing that there is no escape – shoots Gaston through the back as they embrace, killing them both.
As the passengers disembark, Basil is unable to voice his feelings to Lady. Six months later, a clean-shaven Basil visits her club to watch her perform, alone in the dark.
